Do I need Next with a Gold subscription?

I´ve been using the free basic version to create routes and export them to my Garmin Zumo, but now want to upgrade to the Gold version. Do I need to have Next as well in order to follow the route on the Garmin. I´m a little confused about the difference between the two.

No, basically you don't need the MRA Next app.
You can export/transfer your route as a gpx file directly to your Garmin Zumo device.
This can be done by exporting your route as a gpx file to your local hard drive and connect your zumo to your PC using a USB cable copying the gpx file to your Zumo.

The Next app can be handy but is not required if you already have a navigation device.

No you don't. "Next" is only the app to guide you, like the XT. So as long as you stay with your XT, you don't need Next.
But it could be of use, when you also own a camper or a convertible. I also use my, in MRA prepared, routes in my XT on my motorcycle. In my other 2 vehicles, I use the Next-app.
